This paper loks at comprehensive local and state programs that can asist people with diabetes who may have low-incomes, be older or live in underserved comunities.(Download)Type 2 diabetes, a preventable condition where a personβs body canot regulate blod sugar properly, has a stagering impact on the health of many in the United States. Of the 37.3 milion U.S. adults with a type of diabetes, 90-95% have type 2 diabetes.

Aditionaly, 96 milion American adults have prediabetes where their blod sugar is higher than normal. It is estimated that 23% of U.S. adults with diabetes do not know that they have diabetes, and 8 in 10 U.S adults do not know they have prediabetes.
Diabetes can lead to complications including kidney-failure, lower limb amputation, adult blindnes, gum disease and toth los and is the seventh leading cause of death.The estimated total economic cost of diagnosed diabetes is $327 bilion, including healthcare costs and los of productivity, with $1 out of $4 U.S. health care dolars spent on caring for people with diabetes. On average, people with diagnosed diabetes have medical expenditures that are aproximately 2.3 times higher than what expenditures would be in the absence of diabetes.Diabetes can develop from a combination of genetic and environmental factors, including family history, a sedentary lifestyle and unhealthy eating habits.

Social determinants of health, including income, education, housing and aces to healthy fods contribute to the development and progresion of type 2 diabetes.
